VialTalk's catalog lists this blend as "Amylin Analog + Dual Incretin Agonist". Its vendor listing is "LCA-T2/Cagrilintide Blend". In the catalog, "LCA-T2" is a vendor alias of the Dual Incretin Agonist entry, which is tirzepatide, and "Cagrilintide" is the Amylin Analog entry. The blend therefore pairs cagrilintide, a long-acting amylin analog from Novo Nordisk, with tirzepatide, an approved dual GIP/GLP-1 receptor agonist from Eli Lilly. The well-known amylin-plus-incretin combination is cagrilintide with semaglutide (CagriSema). Cagrilintide with tirzepatide is a different pairing. This monograph sets out what each component contributes and what has, and has not, been studied for this combination.
Composition
| Component | Catalog entry | Class | Status |
|---|
| Cagrilintide | Amylin Analog | Long-acting amylin analog; amylin/calcitonin receptor co-agonist | Investigational [1][3] |
| Tirzepatide | Dual Incretin Agonist (alias LCA-T2) | Dual GIP / GLP-1 receptor agonist | FDA-approved (Mounjaro, Zepbound) [2] |
The vendor alias gives a fill per component. Those figures are catalog facts only and are not discussed further here. Verify the actual content of each component on the COA.
What each component contributes
Cagrilintide: the amylin pathway.
- Amylin is co-secreted with insulin. It slows gastric emptying, suppresses glucagon and ends meals through central mechanisms [6].
- Cagrilintide is described as a non-selective amylin receptor agonist [11]. A review contrasts dual amylin/calcitonin agonists with more selective amylin agonists and proposes that calcitonin-receptor engagement may affect tolerability [5].
- In a 26-week Phase 2 trial in which 706 participants were assigned to several exposure levels of cagrilintide (with separate liraglutide and placebo groups), mean weight loss ranged from 6.0% to 10.8%, compared with 3.0% on placebo. The group receiving the most drug lost more weight than the liraglutide group [3].
- Gastrointestinal adverse events were more frequent than on placebo (41–63% vs 32%), mainly nausea [3].
- Cagrilintide is still investigational [1].
Tirzepatide: the dual incretin pathway.
- Tirzepatide activates the GLP-1 and GIP receptors. The contribution of the GIP arm in humans is still being characterised [2].
- In SURMOUNT-1, a 72-week Phase 3 trial of 2,539 adults with obesity and without diabetes, mean weight change was −15.0% to −20.9% across tirzepatide groups, compared with −3.1% on placebo. Cardiometabolic measures improved [4].
- Gastrointestinal adverse events were the most common and occurred mainly early in treatment. Discontinuation because of adverse events was 4.3–7.1%, compared with 2.6% on placebo [4].
Why researchers pair the two pathways. The amylin pathway is separate from the GLP-1 pathway, and that separation is the rationale for studying amylin analogs alongside incretin agonists [1][7]. One review reports promising early-phase data for combining GLP-1 agonists with amylin agonists in general [7]; the best-known amylin-plus-incretin combination is cagrilintide with semaglutide [6].
Combination evidence — the honest boundary
- Human: none for this pairing. A PubMed search for cagrilintide and tirzepatide together returned reviews and network meta-analyses that compare the drugs as separate treatments. One of these ranked tirzepatide and CagriSema (cagrilintide plus semaglutide) close together for weight reduction [9]. That meta-analysis notes that no head-to-head studies have compared all of these medications [9]. We found no trial that gave cagrilintide and tirzepatide together. No human data exist on their combined efficacy, safety, tolerability or pharmacokinetic interaction.
- CagriSema is not this blend. Results from cagrilintide plus semaglutide, a GLP-1-only partner, cannot be transferred directly to cagrilintide plus tirzepatide, which also activates the GIP receptor [2][6].
- Animal: indirect only.
- In diet-induced obese rats, cagrilintide combined with retatrutide reduced body weight more than either drug alone. It also beat comparator combinations that used semaglutide or tirzepatide at matched exposure [10].
- The abstract does not give separate results for a cagrilintide-plus-tirzepatide arm, so the size of any effect from that pairing cannot be inferred from it [10].
- Cell / in vitro: components tested separately. In mouse C2C12 and primary human muscle cells, cagrilintide briefly reduced mitochondrial respiration. Tirzepatide improved maximal respiration by day 5 [8]. The drugs were not combined, and in vitro muscle data do not predict outcomes in whole organisms [8].
- Overlapping adverse effects are untested. Both components on their own cause gastrointestinal adverse events [3][4]. Whether these add up in combination is unknown.
- Formulation is uncharacterised. We found no published data on whether the two peptides are compatible or stable when co-formulated in a single vial.
Research context
Combining amylin and incretin pathways is one of the most active directions in obesity research. Reviews describe long-acting amylin agents and combination approaches as showing clinically meaningful weight loss with generally favourable tolerability [6]. The clinically developed combination is cagrilintide with semaglutide, not with tirzepatide [6][9]. This blend is a vendor-assembled pairing of two compounds from different sponsors. Every published efficacy figure above belongs to one component studied alone. Cagrilintide is investigational [1]. Tirzepatide is a prescription drug [2].
Storage and handling
Research-grade material is supplied as a lyophilized powder. Keep sealed vials cold, dry and protected from light, and follow the batch COA or label. No stability data for this two-peptide co-formulation were found. This monograph does not cover preparation.
Quality and COA considerations
Tirzepatide's molecular mass is about 4814 Da [2]. In a two-peptide vial, a credible COA should:
- confirm the identity of both cagrilintide and tirzepatide by mass spectrometry, ideally LC-MS with resolved peaks
- report purity by HPLC
- quantify each component separately
A single purity value for the whole vial cannot show whether one component is missing or degraded. The VialTalk cagrilintide monograph recommends confirming that the amylin component is cagrilintide specifically and not another amylin analog [1]. That check matters more here because the catalog labels are generic class names.
